Sharp Rees-Stealy partnered with Agile Health to deploy myAgileLife|Welcome Home – a 90-day, interactive text-messaging based coaching program to help engage and support the nearly 163 patients discharging per day within the SRS population. In addition to significant improvement in readmission rates, Welcome Home contributed to:
